Understanding FAMI


While the story of FAMI provides an interesting anecdote to share with friends and prospective patients; a narrative of good timing, coincidence and innovation, the true magic of the groundbreaking technique remains an intricate understanding and harnessing of science.

Built upon years of research and testing, the FAMI technique is a clear example of combining emerging technology, scientific research and experience to solve real-world, medical challenges in a safe and lasting way.

To be able to guarantee such predictable and permanent results, Dr. Amar and his scholars have gone to great lengths to become irrefutable experts in their field, confidently grasping the often-labyrinthine details of the science that anchors their novel approach to restorative surgery.

Having placed such an emphasis on the role of science on the successful implementation of his patented technique, it is not surprising that Dr. Amar would insist on providing a comparable level of explanation and understanding to all those considering entering the world of FAMI.

For Dr. Amar, knowledge and understanding is not only key to his own success and surgical confidence, but also the confidence and comfort of his patients. To develop a healthy and effective medical relationship, Dr. Amar believes it is imperative that a patient be given all the information relevant to the procedure they are about to undergo.

So, over the following chapters, the scientific foundation of the FAMI procedure will be explained in detail, offering the inception, importance and role of each tool and chosen technique.

While much of this is intended to educate and inform, these explanations are also meant to provide context to the ideas that helped lead to the creation of FAMI and honour those scientists whose work made this revolutionary procedure possible. Fig 18

CHAPTER ONE: THE SCIENCE OF ANATOMY IN FAMI
As we established in Section 1, the facial structure is a deeply complex, multilayer natural environment, representing an infinitely challenging setting for anyone who desires a glimpse beneath the surface.

However, for generations of research and study, that surface was the only option of scientists and medical professionals, leaving them with little more than a superficial understanding of this intricate structure that lay just underneath the skin.

Medical texts were almost universally incomplete, offering only rudimentary illustrations and insufficient descriptions, which left surgeons unprepared for the challenges they faced. Fig 19

As Dr. Amar has noted, the available resources supplied only a two-dimensional grasp of the facial area, greatly limiting the scope of surgeons. For that reason, steps forward, like the advent of the FAMI technique were simply impossible to create and more importantly, to test. Fig 20

These limitations meant that cosmetic surgeons entered the operating room each day essentially blind to the complexities beneath the skin, leading to often-unpredictable results.

However, these restraints did not keep people from imagining what was possible. Fortunately for doctors and patients alike, technology eventually caught up with imagination, as the availability of naturally preserved models gave us the opportunity to peer into the natural system of the face in a way never seen before and allowed surgeons to venture into uncharted anatomical regions with confidence. The naturally preserved models that emerged in the late 1990s helped researchers dramatically expand their scope and understanding of the facial network and helped clear the way for Dr. Amar to test his theories related to the safe and effective trans-cutaneous cannulation of the facial muscles.

This represents no small feat for medical professionals given the multiple layers of bone, muscle, nerves and blood vessels they encountered beneath the skin.

Layered atop a continually curving foundation of bone, the face is made up of overlapping layers of muscle. Named for their location, the deep, middle and superficial plane muscle tissue criss-cross the skull, acting in correlation with each other to create facial expressions.

For example, an average human being will use a total of 32 facial muscles to create a content and happy smile. Using eight from one’s deep plane, sixteen from the middle and eight from their superficial, a person’s face will raise their cheeks, curve their mouth and possibly open their mouth wide to reveal their teeth.

The count of functioning muscles varies from smile to smile. As Dr. Amar has observed, the subtle knowing smile of Leonardo da Vinci’s Mona Lisa likely required the use of only two muscles on one side of her face. . Fig 22.

While this expansive knowledge of muscle layers allowed surgeons to explore the facial network with more confidence, it was the deep tissue that most inspired Dr. Amar and the evolution of FAMI.

It was here, within the shifting muscles that lined the skull, that Dr. Amar recognized a broader potential for regenerative cells. Imagining how the extracted cells might be injected into the foundation layer of the facial network, Dr. Amar was then able to visualize his results in a fuller, three dimensional, and thus, more predictable manner.

Further, the ability to explore the neurological and vascular systems that ran within and across the facial muscles in their natural form, allowed Dr. Amar to surmise how he might be able to enter those foundation muscles with the least amount of trauma. Learning to navigate these complex environments would be pivotal to achieving surgery without a scalpel.

Dr. Amar had recognized the importance of understanding anatomy and what role it could play in advancing cosmetic surgery, but this level of exploration and access opened a new chapter in his professional life.
